•  PADI Medical Statement signed and stamped by a physician (no more than 12 months old).
• PADI diving certification details for OWD, AOWD, Rescue Diver, Divemaster and current EFR (not older than 2 years)
• Photocopies of ALL NON PADI diving certification (!)
